We ask that a 50% deposit be made when the order is placed.. Visa, MasterCard and Discover are accepted, balance due the day of the event... NOTE: ALL ORDERS MUST BE SIGNED... Acknowledging He/She has read and agrees to the contract including the Policies Stated below.

AMUSEMENT RENTALS BY J&J, INC. POLICIES

1/ PERSONS RESPONSIBLE: This agreement consists of the printed and written terms of this page and its reverse side. Customer signing this agreement and anyone else for whom the Customer is signing in as a representative capacity, both are jointly and severally responsible for the charges. Officers and directors of a corporate Customer who sign this agreement or any invoice guarantee personally the payment of any sums due Amusement Rentals by J&J, Inc. All Invoices must be signed, indicating method of payment, also any changes to the original order must be requested in writing. We will try our best to honor the request.

2/ CHARGES: Customer accepts the charges that are listed on the reverse side of this agreement. Additional fees will be added if equipment is lost, damaged or returned dirty, wet or late. Reservations are held with a 50% deposit, credit cards are accepted for a security hold, but If order Is canceled there will be a 10% cancellation fee charged to the account. Late fees are charged at 1 1/2 % per month with a $5.00 minimum charge after 30 days. Returned check fee--$28.00 or 20%, which ever is greater. on all returned checks.

3/ AUDIT: All charges are subject to an audit. If an error is found in an earlier calculation, additional billing or refund will be made.

4/ DELIVERY CHARGES: Prices for delivery are quoted for delivery and pickup, one vehicle, one person. The standard rate includes ground floor, tailgate/loading dock delivery. Special request, limited access facilities, or stairs will increase the delivery fee. When placing your order, please explain your particular delivery requirements.

5/ RAIN/CANCELLATION: In case of rain or bad weather, customer must call four (4) Hours prior to the event start time to cancel the event. If this is not done a turnaround fee of $50.00 per vehicle, plus any travel fees charged for events out of our normal service, area will be charged. Once the equipment is setup, even though the weather or other activities prevent the use of the equipment. the full amount of the contract is due.

6/ MISCELLANEOUS:

· All leased food equipment must be cleaned before returning. If not cleaned, there will be a cleaning charge of $25.00 per machine.

· Our staff member that is assigned to your event is to oversee the proper use of the equipment rented. They are not to take tickets or money. We strongly encourage your organization to have people to assist the people using the equipment.

· Each piece of equipment has guidelines for Its proper use. We reserve the right to remove the leased equipment if our staff member encounters problems with persons at the event not following these guidelines. The full amount on contract is due and there will be no refunds in those cases.

·Once the equipment is setup it cannot be moved unless it presents a safety hazard.

·No one under the age of 18 or under 5 feet tall is allowed in the Dunk Tank. The Moon Bounce is classified as a kiddie ride by the insurance carrier, therefore we do not allow the use of the moon-bounce by persons 13 years and older.

· Events outside the normal hours of operation (9:00 AM to 5:00 PM), or outside our regular serving area will be subject to additional charges. Clients requesting early setup times or services not regularly included with the leased equipment will be subject to additional fees.

· If electricity and/or a water source is needed for the leased equipment It is the responsibility of the client. Discuss your requirements with us.

· If leased equipment does not work properly, telephone our office at (703) 450-2792 for assistance. The equipment Is checked out, instructions are given, and is checked upon return. We are not responsible for the weather, other activities at the time of the event, or misunderstood instructions which would Interfere with the use of the equipment. Therefore, no refunds will be given.
